Change "Doesn't meet the standard" icon to a more distinct one

Mahara 16.10RC1

The "Doesn't meet the standard" icon (white circle with red border) is too close to the "Ready for assessment" icon (white circle with blue border). We need something a bit more distinguishable for accessibility and decided to go with the times-circle icon (red circle with white cross).
