Bug Description

Hello.

First thanks a lot for your plugin, which overcomes the closed "k"epub format!

I encountered the following issue: when I click "send to device" button, the selected epub is converted and sent to my kobo reader, but the chapters are cut each time a "page break" is met.
It results in too many chapters, and unnamed chapters.

I attach a sample epub to illustrate and reproduce.

First my details:
- my device: Kobo Glo HD, soft version: 3.19.5761 (0e028f3185, 29/12/2015)
- calibre: 2.54.0
- KoboTouchExtended: 2.6.1

How to reproduce:

- On calibre
    - Import the attached ebook into calibre
    - Connect the device
    - Select the book in Calibre
    - Click the "send to device" button.

- On the reader:
    - Open the book on device.
    - go to the "table des matières" (table of content? list of chapters).
    - click the 19th chapter "Le conseil se sabote"
    - click next page until the last page of this chapter (last words are "son énorme tête s'enfoncer dans les vagues").
        - You can notice that at the bottom of the page we have the chapter name and the current page / total chapter page count.
        - This is not the end of the chapter, but the original epub has a page break here, detected as the end of the current chapter.
    - Click next page
      => First words are "J'ai donné une tape dans le dos de Tyson."
        - remark: it is the expected words (and your plugin is good where other are messed and go back to the beginning of chapter 5 !).
        - BUT: this is not the beginning of a chapter.
        - The chapter name has disappear from the bottom of the page (only the current page and current chapter page count are displayed).

Bref: after each page break, the plugin add a "no name" chapter.

Tip?: in the standard calibre conversion page, under the "structure detection", there is a "chapter mark" combobox with "pagebreak, rule, both, none" choices.
I didn't see any option in your plugin about that. Maybe you could add "ignore page break for chapter detection, if only you are still able to detect them from other ways.

Also, this isn't a plugin issue. Chapter splitting is handled by calibre itself. See Preferences -> Conversion -> Common Options -> Structure Detection.
