Bug Description

I don't have a lot of additional information -- I'm trying to install Ubuntu 16 (latest stable or long term release) on an ext4 partition with Ubuntu 14 and windows 8 on other partitions and it crashes fatally when installing.

Your EFI system partition is corrupt. You will need to either reformat it, or try to repair it using dosfsck in linux, or chkdsk in Windows.
