Bug Description
I was installing Ubuntu 16.04 LTS from optical media as an "reinstall" using a "live" environment - this was because my previous upgrade (from Ubuntu 14.04 LTS to Ubuntu 16.04 LTS resulted in a corrupt kernel)...
Everything seemed to be going well, until the end when the installer crashed.
