Bug Description

I was installing Ubuntu 16.04 LTS from optical media as an "reinstall" using a "live" environment - this was because my previous upgrade (from Ubuntu 14.04 LTS to Ubuntu 16.04 LTS resulted in a corrupt kernel)...

Everything seemed to be going well, until the end when the installer crashed.

You are trying to install in EFI mode to a disk that is not partitioned for it, and chose to ignore the warning suggesting a bios mode install.
