help text for juju bootstrap needs improving

The summary says:

        Usage: juju bootstrap [options] <controllername> <cloud>[/<region>]

It is not clear what the acceptable values for <controllername>
or <cloud> might be. There's no other mention of "controllername"
in the help text, and no evident way to find out what clouds are available.
For example, "lxd" is a valid value for <cloud> but lxd is not the name
of a cloud.
