"Copy Data" checkbox on Image create has the wrong label

How to reproduce:
 - Go to project -> Image
 - Click on "Create Image"

Expected:
 There is a "Copy Data" checkbox

Result:
 There is a "Image Location" checkbox with weird alignment.

Which version of OpenStack are you seeing this, I tried in Mitaka and seems fine. Could you please attach a screenshot of the issue.
