Handle Update of dest IP for Health Check instance

Issue:
------
When there is a change of destination IP in Health Check
Object it was not getting reflected for the existing
running instances, as the metadata IP object was not
updated with the new destination IP for exisiting running
instances

Fix:
----
for existing instances trigger explicit update to metadata
IP object with new destination IP
