pt-heartbeat should include GTIDs in the heartbeat table

Bug Description

To make it easier to monitor and find errant transactions it would be useful to include the Executed Gtid Set in another column in the heartbeat table.

The GTID_SUBSET() and GTID_SUBTRACT() functions could then be executed on the slave(s) to determine the presence of errant transactions.

The GTID information could also be used to determine how far the slave is behind in terms of statements.
