Arrows for re-arranging auth methods are missing

Mahara 15.10dev

The arrows for moving auth methods up and down on the institution edit screen are missing. They are marked up, but don't display.

This is fixed in current master

When I add a second option I get the arrows.

but if I save the institution, navigate away, then edit institution again I don't have the arrows

Bug 1498702: Needing to show arrow icon as button

and fix the deleting of auth instance by deleting the correct div by
count rather than child node by count
