connect-trigger-controller-to-app

Currently automated trigger handling, such as those received from github webhooks, is associated with plans and assemblies.
Specifically, trigger_id (and trust_id) are associated with plan.
Per the app and workflow spec [1], we want to change this and move trigger handling functionality to app and workflow resources.
