Bug Description

Fuel ISO#521 (upgraded)
OS: Ubuntu, CentOS

Description:

Deployment failed after upgrade fuel from 6.0 to 6.1 with error 'Too many nodes failed to provision'.

Steps to reproduce:

1. Fuel 6.0:
   Create new enviroment with following parameters:
      HA mode is on
      Compute: QEMU
      Networking Setup: Nova-network
      Storage backends: Cinder - default, Glance - default
      Additional services: all by default

2. Add 1 controller and deploy env.
3. Upgrade fuel from 6.0 to 6.1
4. Try to create new env with any configuration.

Observed behaviour:
Deployment is failed with error 'Too many nodes failed to provision'.

Can't reproduce this bug on RC3 (ISO #525).
