Sound to USB speakers doesn't resume after suspend

Bug Description
Lenovo x220 i5, with external USB speakers. When the laptop wakes from sleep, there is no sound, either from the internal speakers or the attached USB speakers. It is not a mute issue. The USB speakers must be unplugged and reinserted for sound to resume. In addition "Analog Output" must be selected in the Sound Settings menu before the hardware buttons will work again for sound.
