I can reproduce this crash easily trying to reformat a USB drive:
1) Insert USB drive
2) Open Disk Utility and format with MBR disk label
3) Click "+" button to create partition
4) Click "Create"

$ lsb_release -a
No LSB modules are available.
Distributor ID: Ubuntu

I got the same issue while creating a partition on a normal sata connection.

I got this crash on xenial 16.04 today when I tried to power off a USB device that remained after I removed it from the system. I can reproduce this on trusty as well on another Ubuntu system with trusty 14.04.4 LTS.
