Tried to deploy the following configuration:
Baremetal,Ubuntu, HA, Neutron-vlan,Ceph-all, Debug, nova-quotas, image-based,6.1-192
Controllers:3 Computes:48

I chose image-based provisioning
One node (node-8) wasn't been provisioned. (screenshot is attached)

In cobbler netboot_enabled=false for this node, therefore even after reboot the node wasn't booted in debian installer image

definately dpulicates https://bugs.launchpad.net/fuel/+bug/1420256

and I thought that original bug was fixed https://bugs.launchpad.net/fuel/+bug/1427718 (fix was landed on Mar 18, 2015 21:14)

could you retry deploy on more recent iso ?
