middleware and profiler notifications generate non-metric meters

the http.* and profiler.* meters were missed as part of transition to enforce meters having a non-hardcoded measurement value

    support disabling profiler and http meters

    there is support to disable all non-metric meters. the profiler and
    http meters should be part of this list but were left out.
