When using Load MARC Order Records the Provider field is required but the interface does not indicate this and if left blank on upload the user receives a Database Query Failed message with no indication as to what caused the error. Upload button should not activate unless provider field contains data. Related bug #1363256 - Dojo edit forms need to indicate required fields.

As of 3.10 and the angular Load MARC Order Records, Provider is now a required field and the form cannot be submitted without it. Marking Won't Fix.
