a2x does not find icons when creating an epub

Bug Description

When trying to create an epub using the asciidoc a2x utility there is an error about missing icons. These icons
are on the path /usr/share/xml/docbook/stylesheet/docbook-xsl/images but the directory must be copied
locally and passed as --icons-dir for a2x to successfully generate an epub.

This problem is documented here: http://www.methods.co.nz/asciidoc/epub-notes.txt under the heading
"=== epub: Unreferenced callout icons in OPF" with a note that a workaround was implemented in version 8.6.5 of a2x.
Ubuntu 14.10 currently installs 8.6.9 so this workaround appears to be broken now.

I am unable to reproduce this problem with the latest version (8.6.9-5).
