[Dasboard] It's not clear for a user what to do in a 2d step of package upload

1) Got to Murano->Manage->Package Definition tab
2) Try to add simple application package
3) Upload a file and click next

Package parameters appeared. User can review them. But without providing a category package would not be upload

Need to make it clear for a user what he required to do on this step.
Or add another step with category selection and allow adding new category

    Make category selection a separate step

    After package file is upload, user should point to witch category this package belongs.
    Now, it's not obvious that this parameter is requred and user don't undestand
    what he needs to do. Making a separate step for category selection will make that clear.
    Also, templates for that forms were updated.
