Guided partitioning
Guided - use entire disk and set up LVM

Select disk to partition:
SCSI3 (0,0,0) (sda) - 1.5 TB AMCC 9650SE-4LP DISK

The following partitions are going to be formatted:
   partition #1 of SCSI3 (0,0,0) (sda) as ext3

ERROR!
[!!] Partition disks
Unexpected error while creating volume group
Autopartitioning using LVM failed because an error occurred while creating the volume group.

Can you please run the installer again and attach /var/log/partman and /var/log/syslog from the installation environment after you encounter this error?

Changed in partman-lvm:
status: New → Incomplete

partman: No matching physical volumes found
partman: No volume groups found
partman: Reading all physical volumes. This make take a while.
partman-lvm: Physical volume '/dev/sda5' is already in volume group 'barracudas'
partman-lvm: Unable to add physical volume '/dev/sda5' to volume group 'heretic'

So it appears that when Autopartitioning with LVM is attempted, there is no check to see if a LVM volume group already exist on the drive. And if a volume group is found on the drive, the creation of the new volume group can not be done, and an Unexpected error is thrown.
