update/cleanup to bootstrap 3 icons

There are still some instances of "<i class="icon-ok"></i>" and "glyphicon glyphicon-____' for icons.

We should be using font-awesome instead.

thx, will do. I have the latest code and tried embedding '<i class="fa fa-wifi"></i>' to test, but it didn't work...
