"Save Settings" button becomes disabled after checking "Install Mellanox drivers and SR-IOV plugin"

Steps to reproduce:

1. Create environment with Neutron with VLAN segmentation and KVM hypervisor
2. Check "Install Mellanox drivers and SR-IOV plugin" radiobutton

Result: "Save Settings" button becomes disabled and stays and stays at that state even after another changes. There is also Javascript error "TypeError: Function.prototype.bind called on incompatible undefined"
