[docs] Document the upgrade procedure for environments deployed by Fuel

Fuel 5.0.1 is the first release that allows customers to upgrade rather than having to completely redeploy. We need to provide instructions, probably in http://docs.mirantis.com/openstack/fuel/master/user-guide.html
