Refusing to bootstrap mysql cluster until role is known. (missing heat metadata)

As of today I'm seeing repeated undercloud job failures due with the following in the os-collect-config log file:

We are neither cluster initializer or joiner. Refusing to bootstrap mysql cluster until role is known.

The root cause of the issue appears to be related to missing metadata. I'm seeing this in the Nova API logs:
