In To install and configure the components section, number 2b is missing the equals sign:

[database]
...
connection mysql://keystone:KEYSTONE_DBPASS@controller/keystone

should read...

[database]
...
connection = mysql://keystone:KEYSTONE_DBPASS@controller/keystone
