resolve documentation links for libncurses/libncursesw

Editions of LSB prior to 5.0 referenced X/Open Curses Issue 4 Version 2,
providing relative links in the interface record (now in the IntStd entry,
actually), off a base link for that specification on the Open Group website.
For LSB 5.0, this reference has been changed to Issue 7. However, at this time,
neither 4.2 or 7 are available as a broken-out on-line html document (the
former was, at one time). The Standard record for curses in either case refers
to a document which may be downloaded without charge, so in terms of the LSB
specification itself, this is not invalid. However, LSB Navigator likes to
have the capability of jumping to the on-line documentation for an interface,
and this is now not possible. We need to come to a resolution for the
documentation links.

Meanwhile, libncursesw (new to 5.0), while in large part identical in
specification to libcurses, does not reference the X/Open Curses specifications
at all. Instead, it references the ncurses upstream site. In this case, we
actually have working documentation links in Navigator, but we don't have a
base specification document (the ncurses site is Documentation but not
Specification, in LSB terminology) for the library. We need to finish
resolving that as well. When the ncursesw import was done, a second entry was
made entitled "Libncursesw Specification Placeholder" in anticipation of
putting something there.
