Bug Description

I cannot boot into Windows after a clean install upgrade to 14.04 LTS

The windows bootloader option does not work both in Grub and in the Bios.

The problem is not fixed even after running boot repair on a live usb.

In order to proceed we need a better description that "doesn't work", as well as the output of sudo efibootmgr -v when the system is in this state.
