floating-ip-list does not list attached server anymore

https://review.openstack.org/#/c/47859/ changed v1_1/shell.py from "Instance Id" to "Server Id". However the field in the V1.1 api data is still called "instance_id". There is no corresponding "server_id" field.

This causes nova floating-ip-list to show always an empty output in the "Server id" column.
