PulseAudio crash via "an application crashed . . ."

Bug Description

I received the notification everytime a log into a newly installed not upgraded Kubuntu 14.04 and sometimes during a session. I kept getting messages about a component failing and PulseAudio going to "Default". I went into systemsettings/multimedia and changed the default sound device to default. Now instead of 'default' notifications, I get crash reports.
