backport dmraid45 patches for trsuty hwe kernel, since trusty kernel has dmraid45 disabled.

I didn't think these patches were interoperable among different kernel versions? In other words, once dmraid is patched to use the new module, it no longer will work on a system booted with any of the other precise kernels.

The Precise Pangolin has reached end of life, so this bug will not be fixed for that release
