Catalog needs more format icons

With the recent MVF work, Evergreen now supports the representation by icons of many more formats, but doesn't have enough icons for each of those formats. We have a set of new icons to represent all the new formats available in the seed data.

* In a couple of cases, we created an icon without realizing that the format already had an available icon in Evergreen. I included those icons with a name like score2.png so that the icons wouldn't replace the current icons, but would be easily accessible for anyone who might want to use something that's a little different from the stock icons.

* We also included a couple of individual icons for cd, cassette, phono that were used as the basis for format-specific audiobook and music icons. They might come in handy if an Evergreen site chooses to use broader format categories for their icons.

Many thanks to Jim Keenan at C/W MARS for creating many of these icons with some minor adaptations from me.

My hope is that the additional icons might be seen as a bug fix for 2.6 since the new icon formats aren't very useful if the system isn't using distinct graphic images.
