Unity Top-Menu Bar Not Showing Window Control Buttons

This is similar to Bug ​ #842566, but with an additional issue.

When openinf LibreOffice, Window Controls do not appear on mouse-over in the Unity Top-Menu Bar.

Also the LibreOffice window header is lost beneath the Unity Bar

I have the same problem, but oddly enough in Chromium - LibreOffice is OK. So I don't think this is related to LibreOffice.
This only happens when maximised.

I have the same problem with gtaphic card intel (Intel Ivybridge Mobile) and writer.

It happened to me when spawning a window with gtk+ 2.0 and chromium embedded framework.
