Centos addresses hard drives
disk/by-path/pci-0000:02:00.0-scsi-0:0:0:0
disk/by-path/pci-0000:02:00.0-scsi-0:0:0:1
and ubuntu
disk/by-path/pci-0000:02:00.0-scsi-0:0:0:1
disk/by-path/pci-0000:02:00.0-scsi-0:0:0:2

need to use wwn instead of path

    Added extra field into node disks

    Extra parameters, which actually are the id names
    of hard drives, allow to find them under ubuntu
    kernel.

    Pmanager looks for disk devices via id

    This patch is to avoid the situation when
    bootstrap centos kernel creates hard drive devices which
    then can not be found under ubuntu kernel.

It looks like in some cases we still do incorrect partition number calculations for GRUB to install.
Attached logs and info from ticket 1312 show it.
