What's the package version in the build? I suspect an OS patch is missing, either on lsb-release or on software-center.

As discussed in IRC, software-center is 5.2.10+elementary13 and lsb-release
is 4.0-0ubuntu20.3+elementary5

my software-center closes after opening a few seconds, have no idea if it related to lsb-release or not, but my console output looks like Cody Garver's.

It appears that the patched Python lsb-release module doesn't return "CODENAME" for some reason...

The simplest fault would be incorrect values in /etc/upstream-release/lsb-release or in /etc/lsb-release; the more complicated reason would be an out-of-date patch on lsb-release Python module.

Please provide me with a fresh ISO build so I could debug the issue.

This looks like an outdated base-files OS patch to me now. Downloading the ISO to verify.
