1. Create new environment (CentOS, simple mode)
2. Choose Ceph volumes and Savanna
3. Add controller, compute, 2 ceph nodes
4. Choose VLAN Manager in network settings
5. Start deployment. It was successful
6. Login into Horizon
7. Move to tab "Savanna"
8. Try to create cluster template
Error: Internal Server Error

there are no 500 errors from savanna or dashboard in logs. this bug is absolutely incomplete

For the record, I run into the same problem. I'm also using Fuel 4.0 (on Virtualbox on Ubuntu). I have tried several different environments using VLAN/GRE CentOS/Ubuntu etc. I can upload and mark the Hadoop 1.2.1 Ubuntu QCOW2 image; I can create the Node Group template; but I am not able to create a cluster template. I receive the same error.

Never mind. I was able to get it to work now. Apparently I was missing the jobtracker in the Node Groups.
