VMware: "Unable to access file" VC error when trying to attach vmdk volume to VM

Bug Description

In a slow VC setup where the backing creation takes more than 60 secs, the rpc_response_timeout is exceeded and the cinder-api tries to initialize_connection a second time. This causes the vmdk driver to issue a second create_backing call to the VC. This resulted in both the create_backing calls failing out with VC's "Unable to access file" error. See attached VC task screen capture and the corresponding cinder logs.

Please provide more details how to reproduce this.
Also, if the first createVm_Task is running, only the second task will fail, right?
