Make revisions history for the release docs PDF

We need to introduce revisions history in release notes, as we may need to update them during the release cycle.
For example, we need to update release notes for 3.2.1 with the known issue with security groups. As we do the update, we must add a line of history for the doc in the following example format:
Revision Date Description
3.2.1 5-Dec-13 Initial GA
3.2.1 v1 9-Dec-13 Added known issue, "Node ignites under too much power"
3.2.1 v2 17-Dec-13 Added known issue, "Fuel sometimes removes money directly from customer bank account"

Is it really bug? Looks like it is guideline to update release notes

Implemented GA version here: http://docs.mirantis.com/fuel/fuel-4.0/pdf/Mirantis-OpenStack-4.0-RelNotes.pdf.
For other modifications of release notes, we need to put a new version there. So let's close this bug.
