Bug Description
Hi Devs,
I have a fairly simple parallel adaptive test case (2D, prescribed velocity only), which is failing in zoltan during its first adapt. An error message is printed on 1 core, which states:
4 Sanity check failed: col_hindex[
I have attached everything that is necessary to reproduce the bug to this ticket. Is there any chance somebody could take a look as it's far beyond my expertiese?
FYI - I am on the latest version of trunk R4297 (which has the zoltan twerks branch removed).

Hi Devs - is there any chance that somebody could confirm whether or not this bug is reproducible? I've recently seen it on a few additional (larger) cases, so would be good to know if anybody has any insights/tips...
As this is the smallest problem I've seen the issue with, it may be a good one for debugging.
