Serials: further routing slip improvements

For Evergreen master:

This improves upon bug 1229349 by implementing the changes Dan Wells suggested there:

      - Don't repeat the same address for the same user
      - Label mailing vs. billing addresses
      - Border to separate mailing and billing addresses
      - [Also] move some inline styles to a CSS file
