cookie_name field should be displayed only when session_persistence is APP_COOKIE in LBaaS VIP

Bug Description
When LBaaS VIP creation, cookie_name is required when session_persistence is APP_COOKIE, however in LBaaS VIP creation workflow "Cookie Name" field is always displayed regardless of session_persistence type. It is better to hide it if session_persistence type is not APP_COOKIE.
