Bug Description
In Havana, neutron add a support of service provider of LBaaS.
"service provider" specifies a backend on which LBaaS pool is created.
A user can specify a service provider name when creating a LBaaS pool.
It is useful to have support of LBaaS service provider in Horizon.
To support this, the following need to be added:
- A provider name field in LBaaS pool creation form. Available provider names are retrieved from Neutron.
- A provider name field in LBaaS pool detail (and tables)
