Bug Description
I have had this happen on a few occasions, and it can be quite frustrating.
Make a few phone calls, make sure phone and contacts apps are open as is typical during a day of use, note that after a call you are left in the "call log" screen.
Switch to the contacts app.
Select a contact and make a call to one of their numbers.
At this point you get taken to the call log screen, not the keypad screen. As a result it's _really_ hard to end the call. I found myself swiping in the launcher, clicking the phone app (which did nothing because I'm already in it, looking at the call log). I tried swiping away and then back, and tried tapping tabs in the app. No amount of navigation would get me back to the keypad screen.
This also happened when someone called me, so it's not limited to outbound calls.

Ok, this is easy to reproduce.
Open Dialer
Open Contacts
Call a contact once. It will switch to call log screen, then to the call screen.
Hangup
Call the contact again. It will switch to the call log screen, but not the call screen. The call will still be occurring.
Note, you may have to do the loop several times to get it to glitch.
