Better layout of test submit page

Bug Description

On a test report submitting page, the 'Submit results' and 'Add more bugs' are located next to each other which is a bit confusing. It looks like the submit button is related to the bugs list. I think moving the submit button to the right side of the page and making 'Add more bugs' a text link instead of a button would help.

Also, the text should be 'More bug entry fields' for clarity. I think 3 bug boxes is enough since you can add more.

The normal/serious bug setting should be just a check box that you check for serious. Heading 'Serious'.

Will be fixed for Tribe-3 at the same time as the optional comment and PASS/FAIL option.

Implemented (the "More bug entry fields" is still a button for html limitation reason (I don't want to use javascript just for that))
