pref_ou can repeat indefinitely in recent searches list

In staff client, set the Preferred Library in Search Preferences to something other than the consortium root node. Search for something, and the Recent searches -list should show the search with pref_ou(foo) added to it. Click on that search in the list, and it will add yet another pref_ou(foo). Repeat ad nauseam.

I've also seen filter_group_entry() repeating in the same manner.

Fix for repeating pref_ou in user/paxed/lp1185680 - I couldn't get the filter_group_entry() to repeat in my tests this time, so didn't fix that.
