Missing keyword args from several raise exception calls

I found several places in the code where calls to raise exceptions do not supply the appropriate keyword args. This might mean that an exception would not be raised correctly, or perhaps produce a misleading message.

    Fixes for mis-use of various exceptions

    Changes to various the way various exceptions are raised. They are
    all missing keyword args.
