mouse cursor size setting has no impact on the default cursor size in most apps

Bug Description

Replication:

1. Run xfce4-settings-manager
2. Select "Mouse and Touchpad"
3. Select the "Theme" tab
4. Change the cursor size from 24 to 48.

Expected Result: The cursor size should change to double.

Actual result:
1. The default pointer size doesn't change.
2. The mouse-over cursor size does as expected change to double
             ex: mouse over the cursor size input box and you'll see a double sized text selection cursor.
3. The cursor size in some apps (example, Firefox) does change as expected.
