PostgreSQL PPA for Ubuntu Lucid

All of the README installation guides for 2.2, 2.3, and 2.4beta point to this PPA to install Postgres: ppa:pitti/postgresql -- https://launchpad.net/~pitti/+archive/postgresql

This PPA is going to be deprecated sometime during Ubuntu 12.04’s lifetime. The page says to consider moving to apt.postgresql.org.

Installation instructions should be tested and updated to reflect a change of this PPA.

I've made the changes so that the install docs can use the official PostgreSQL repo to install 9.1.

In the process of installing on a Lucid VM, I discovered an issue with 'Makefile.ubuntu-lucid' that I also worked out.

Alas, with Trusty 14.04 released and the community shifting resources towards adopting that and keeping Precise 12.04, it seems to me that Lucid 10.04 is going to dropped from being a supported platform.
