Looks like broken packages. Try reinstalling the os with new media. Should fix it

also try booting into recovery mode and enable networking and choose the option to repair packages
should fix that too but probably better for a clean install

i was able to get the bug to happen again i just dont have a clue why it happens.

Jack: that last comment was unnecessary, please just use the "confirmed" option unless you can show how to reproduce the bug.
