ubuntu 12.04 unity-launcher display two icons when I start my qt app

when I run my qt app, two icons display on the launcher.
My friend and I try three systems below:
ubuntu 12.04 qt version: 4.7.4, 4.8.0, 4.8.1, 4.8.4,5.0.1
ubuntu 11.10 qt version:4.8.4
ubuntu 12.10 qt version:4.8.3

uname -a
Linux rock-Veriton-X2610 3.2.0-38-generic #61-Ubuntu SMP Tue Feb 19 12:18:21 UTC 2013 x86_64 x86_64 x86_64 GNU/Linux
lsb_release -rd
Description: Ubuntu 12.04.2 LTS

I find LibreOffice also has this problem. I download cppcheck source code and compile it ,it also has this problem.
