Bug Description
I clicked on my gvim icon in the panel to switch between windows, but may click was probably a small drag. This has created an extra gvim icon above the panel (see screenshot) which did not disappear when the gvim application was closed. It was not possible to remove this extra icon. The version of unity is whatever is in the latest updates for Ubuntu 12.04 Precise (Feb 12, 2013).

I used to see this issue in 12.04 as well but have not faced it after. I guess it was fixed sometime between.
