Form Storage Failed Upgrade on 4.1.5

If verified, this is a critical bug as it will prevent updating of iHRIS to 4.1.5

fixed in 4.1-dev with revision 3126:

will be a part of the 4.1.6 release.

I tried to install 4.1.4 and then updated/upgraded that to 4.1.5 and the process produced no errors.
It should ideally be ok. Maybe the problem was because the 4.1.4 instance had data in it!

If this was on a dump of Botswana data, than you should be able to ignore this.
